This application process can only be used if you are registered as a midwife in Australia.

Please read the application guidelines for assistance while completing the application form.  The guidelines provide an overview of the process, explanations about the information required in the application form and information about the supporting information required.

You can complete part of the process and pay by credit card here but some additional documents are required. You should arrange for these to be sent to the secretariat as soon as possible. You should also arrange for your your payment of the application fee if you elect not to pay by credit card. Please note that your application will not be considered by Council until all required documents and the fee have been received.

The Application fee is $NZ500.  This includes New Zealand Goods and Services Tax (GST). 

The following documents are required from all applicants to complete the application process and are additional to the online application form:

  • A statutory declaration
  • Evidence of your identity - certified as a true copy
  • Evidence of current registration and entitlement to practise as a midwife in Australia
  • Details of your recent midwifery practise

The required statutory declaration form will be emailed to you after you submit your application, along with the details of your application. You can also download these forms here.
