Changing Your Name or Address
The Health Practitioner Competence Assurance Act 2003 requires midwives who change their name or their address to promptly notify the Council. It also stipulates that the Council holds more comprehensive address details than has been the custom in the past. This requirement continues to apply so long as you remain on the Register, even if you have stopped working as a midwife.
If you have changed your address you should go to the My Details section and change your address there. This requires you to logon. If you are not able to login please contact the Council.
We cannot alter a name on the Register without formal evidence of a legal name change. If you have changed your name please send us a certified copy of your marriage certificate, deed poll, or statutory declaration, with your request. A photocopy of any document must be certified as a true copy by a Justice of the Peace, Notary Public, or Solicitor (no one else).
